Trabeculectomy for Glaucoma

Trabeculectomy is a surgical operation which lowers the intraocular pressure inside the eye (IOP) in patients with glaucoma. This is achieved by making a small hole in the eye wall (sclera), covered by a thin trap-door in the sclera. The fluid inside the eye known as aqueous humour, drains through the trap-door to a small reservoir or bleb just under the eye surface, hidden by the eyelid. The trap-door is sutured (stitched) in a way that prevents aqueous humour from draining too quickly. By draining aqueous humour the trabeculectomy operation reduces the pressure on the optic nerve and prevents or slows further damage and further loss of vision in glaucoma.

Control of the eye pressure with a trabeculectomy will not restore vision already lost from glaucoma.

After surgery

Straight after your operation a clear plastic shield will be placed over your eye to protect it from accidental knocks.

You will be able to go home and should be given 2 sets of eye drops, usually an antibiotic (such as chloramphenicol) and an anti-inflammatory steroid (such as dexamethasone or predforte).

You will need to come back the next day so the doctor can check your eye after the operation.

You may get blurred or double vision which might last for up to 1 month. Your eye may feel uncomfortable and gritty after your operation.

Wear the eye shield in bed for the next 14 nights. Wash the eye shield daily with hot soap and water.

If you wish, you can wear sunglasses or normal glasses during the day. If your eye is sticky in the mornings, you can bathe it with cooled boiled water and cotton wool.

Cooking, watching TV and reading will not cause any harm. Avoid energetic activity such as contact sports and swimming for 1 month, and heavy gardening and lifting for 4 weeks after your operation.
